Assurant dental plans are underwritten by Sunlife. If your Assurant plan is administered through Sunlife we will accept it.

We are an in-network provider for Sunlife and can file your benefits accordingly.

Please be advised that while we may not be in-network providers for your specific insurance plan, this does not necessarily mean your insurance will not provide coverage. it simply means we are considered out-of-network. We strongly recommend that you verify your insurance benefits and coverage with your provider prior to your appointment, as it is the patients responsibility to ensure services are covered and accepted at our office. As courtesy, we are happy to file claims for all insurance plans with the exception of DMO, HMO and savings plans.
